Parents and athletes,
We are required each year to have documentation by the State of Oklahoma for athletic participation for our students in the 7th-12th grade. Each athlete/parent is required to have a yearly physical on file, been given information on concussions and sudden cardiac arrest and complete an OSSAA eligibility form. To help cut down on the amount of paperwork our coaches have to keep up with, we are solely using an online portal to handle all of our paperwork except physicals. For your student to be eligible to participate in any sport, parents must go online and electronically fill out the information. In addition to signing for the concussion and cardiac arrest forms, we also are asking you to fill out a medical history form and the OSSAA eligibility form. Again, we are required to have these forms on file each year.
Here the steps to get an account and to electronically fill out forms.

You may create you a parent account so you can track your child or you may go straight to filling out the forms.
If you have more than one child, you will be able to add multiple children to your account.
Each child will have to have the forms filled out separately.
The forms tab is at the tab and you have to fill out the OSSAA Eligibility form, the Impact Permission slip, the medical release form, and the athletics participants form which includes the concussion form and the cardiac arrest form.
On the tab next to the electronic forms tab, there is a tab about information. Click this tab to get more information in regards to concussions and cardiac arrest.
All forms will require you to put your child's name and date of birth on them. If you do not enter any information correctly, it will not work. Make sure all spellings of names and dates of birth are correct.
All forms have to be electronically signed by the athlete and a parent.
The only paperwork that has to be filled out and turned into the coach is the physical form.
NO paper copies of the OSSAA eligibility form, medical history, concussion or cardiac arrest will be given out this year. It must be done online. Your child is not eligible until all forms are filled out.
If you do not have access to a computer or the internet, you may come by the school and use our computers to fill out the forms.
